C-46 | In-Hospital Mortality in Patients With Pulmonary Embolism Managed With Ultrasound-Assisted Catheter-Directed Thrombolysis Compared to Systemic Thrombolysis

چکیده
Patients with high-risk or intermediate-risk pulmonary embolism (PE) require more aggressive therapy, such as systemic thrombolysis. Although parenteral thrombolysis leads to faster clot resolution, it is associated a high risk for significant bleeding and its complications. The use of catheter-directed (CDT) decreases exposure thrombolytics reduces the complications mortality. Adding ultrasound CDT can potentially decrease dosing local make even safer patients. This retrospective cohort study identified 1,007 patients diagnosed across several facilities. were stratified into two groups based on treatment they received, group that received (ST), 432 (42.9 %), ultrasound-assisted (EKOS), 575 (57.1 %). aim was explore immediate mortality from direct indirect arising PE management in treated vs. EKOS. We used logistic regression analyze data. who managed ST likely expire than those EKOS (OR 2.893, CI 1.766∼5.041, p<0.001). passed experience tachycardia 2.432, 1,299∼4.556, p<0.001) vasopressors 13.331, 8.075∼22.006, There 70 (6.95 %) deaths 26 (2.58 group. are die illustrates better outcomes using PE. However, limitation this small sample possible cofounders, which warrants future studies.

Ultrasound-assisted catheter-directed thrombolysis for pulmonary embolism
We have read through the case report article with great interest, entitled “Combined catheter thrombus fragmentation and percutaneous thrombectomy in a patient with massive pulmonary emboli and acute cerebral infarct,” by Uğurlu et al. (1) and published in Anatol J Cardiol 2015; 15: 69-74. For the last two years, ultrasound-assisted catheter-directed thrombolytic (USAT) has been used as an alte...
متن کاملRandomized, controlled trial of ultrasound-assisted catheter-directed thrombolysis for acute intermediate-risk pulmonary embolism.
BACKGROUND In patients with acute pulmonary embolism, systemic thrombolysis improves right ventricular (RV) dilatation, is associated with major bleeding, and is withheld in many patients at risk. This multicenter randomized, controlled trial investigated whether ultrasound-assisted catheter-directed thrombolysis (USAT) is superior to anticoagulation alone in the reversal of RV dilatation in in...
متن کاملUltrasound Assisted Catheter-Directed Thrombolysis of Acute Pulmonary Embolism: A Review of Current Literature
Pulmonary embolism continues as a very common and also presumably life-threatening disorder. For affected individuals with intermediate- as well as high-risk pulmonary embolism, catheter-based revascularization procedures have developed a possible substitute for systemic thrombolysis or for surgical embolectomy. Ultrasound-assisted catheter-directed thrombolysis is an innovative catheter-based ...
متن کاملFixed low-dose ultrasound-assisted catheter-directed thrombolysis for intermediate and high-risk pulmonary embolism.
AIMS No standardized local thrombolysis regimen exists for the treatment of pulmonary embolism (PE). We retrospectively investigated efficacy and safety of fixed low-dose ultrasound-assisted catheter-directed thrombolysis (USAT) for intermediate- and high-risk PE. METHODS AND RESULTS Fifty-two patients (65 ± 14 years) of whom 14 had high-risk PE (troponin positive in all) and 38 intermediate-...
متن کاملPrognostic Impact of Thrombolysis in Myocardial Infarction Risk Index on Hospitalization Mortality of Patient with Acute Pulmonary Embolism
Introduction: Acute pulmonary embolism (PE) is one of the deadly cardiovascular diseases. One of the indexes proposed in these patients for risk stratification is the Thrombolysis in Myocardial Infarction (TIMI) risk index (TRI), which includes three parameters of systolic blood pressure, age, and heart rate. This study aimed to evaluate the predictive value of TRI on in-hospit...
